St. Rose Offers New, No Cost Program
(HENDERSON, NV, July 1, 2008) - The Community Wellness office at St. Rose Dominican Hospitals, the community's only not-for-profit, religiously sponsored hospital system, is partnering with physicians across the valley to offer a convenient, no cost congestive heart failure management program.
"St. Rose understands the commitment it takes to properly manage congestive heart failure," said Rod Davis, president, St. Rose Dominican Hospitals. "Management of congestive heart failure helps patients reduce emergency room visits, hospitalizations and unscheduled office visits. This is why we're collaborating with physicians to offer a useful monitoring program for patients at no charge."
Qualified participants:
- are diagnosed with congestive heart failure;
- are under the care of a Nevada licensed physician;
- are available for daily/weekly monitoring, as needed; and
- authorize St. Rose to communicate with their physician.
The individual programs, designed for each patient, are based on open communication between St. Rose and the participant's physician, and the process involves 1) the patient first being evaluated by a St. Rose Home Health nurse, 2) results from the initial evaluation are then shared with and approved by the patient’s physician, 3) blood pressure, body weight, heart rate and SPO2 are measured on a regular basis through the nationally-recognized Honeywell HomMed System at convenient locations; and 4) results from the visits are then transferred and reviewed by a St. Rose nurse and forwarded to the physician.
