Case Managers Help Patients Meet Their Healthcare Needs
At St. Rose Dominican Hospitals, Case Management brings the continuum of care concept to life. Case managers are registered nurses who follow patients from admission to discharge.
One of their main functions is to work in collaboration with the patient and the entire healthcare team to establish an individualized treatment and discharge plan. This may include evaluating options and services to meet each patient's healthcare needs. They work closely with physicians and other nurses to ensure that each patient is receiving the best level of care for their situation and that their transition from discharge to outpatient status is a smooth one.
Case management staff are active not only in the Emergency and Admitting Departments but on each hospital floor. "We want to ensure that the healthcare delivery process is streamlined to focus on the best treatment or approach for our patients," said Lorraine Cenatiempo, Director of Case Management.
Case Managers also make referrals to Social Workers, integral and specialized members of the Case Management Team who make evaluations for issues such as patient and family conferences, community resources, crisis intervention and long-term care applications. In addition, social workers strive to ensure access to needed information on community services and resources that will enhance the patient's opportunity to change or address their own needs.
